Objective: 1. Develop an umbrella program for multi-sites on an agency, state or local level. 2. Implement a program that can gain recognition and allow each site to bill for the service. 3. Maintain recognition through proper technical assistance, program evaluation and on-going audit

Methods: Started a state housed "umbrella" ADA recognized program which was the first of its kind. This program started with 4 local health departments in 2007, added 10 in 2008 and another 14 in 2009. As we move into year 4 we expect to be working with more than 40 local health departments across NC to enhance public health infrastructure, increase diabetes education across the state, and establish systems which will allow local health departments to bring in revenue from insurers.

Results: As we grow we are able to serve more and more patients who have diabetes and wouldn't have had access to care.

Conclusion: This program has been a successful way to increase access and decrease the burden of diabetes.
