Objective: • Demonstrate the ability of the Colorado Diabetes Prevention and Control Program (at the Colorado Department of Public Health and Environment) and the University of Colorado Denver School of Pharmacy to implement a Diabetes Self Management Education (DSME) program to reach people with diabetes at high risk for complications in underserved areas of the state. • Demonstrate the effectiveness of a web-based continuing pharmacy education (CPE) program to prepare pharmacists to deliver DSME in rural areas of the state.
Methods: • Ten pharmacies will be targeted in high-risk, underserved areas of the state of Colorado • Data collection tool will track time spent on CPE, credits earned, and evaluate pharmacists proficiency • Pharmacists will report on program satisfaction and the ability to conduct DSME in rural (high-risk targeted) area of Colorado
Results: In progress; project will be completed 3/31/10
Conclusion: Partnerships with public health departments and schools of pharmacy are an effective means to establish and deliver diabetes self-management education programs
