Biographical Sketch:
Current Position: Senior Program Coordinator West Virginia University Department of Community Medicine – Morgantown, WV Office of Health Services Research July, 2003 – Present • Assist primary care sites in the implementation and integration of electronic patient registries and electronic medical records in quality improvement efforts • Provide technical support for 50+ care sites using electronic patient registries and electronic medical records to track and target care among patients with diabetes, cardiovascular health conditions, asthma, black lung, depression, and other chronic conditions • Health Resources and Services Administration (HRSA) Information Support Agent for all West Virginia health centers in the HRSA National Health Disparities Collaborative • Provide in-person and remote training sessions for patient registry and electronic medical record users • Build ad-hoc patient registry reports for participating health centers • Assist with patient registry customizations and modifications • Assist with the construction of data import/export tools to improve patient registry and electronic medical record functionality • Assist in the development of a web-based dashboard for health outcomes • Assist with the development of web-based surveys for the West Virginia Primary Care Association • Coordinate and provide leadership for the National Chronic Disease Electronic Management System Registry Support Group, which meets monthly via webinars • Assisted in modifying the Chronic Disease Electronic Management System (CDEMS) for use by Johns Hopkins University Diabetes Center. • Co-chairperson of the West Virginia Asthma Coalition’s Clinical Asthma Management Subcommittee • Member of the West Virginia Diabetes Prevention and Control Program Advisory Committee • Fulfill ad-hoc data requests as per office’s role as a state data center for the US Bureau of the Census Publications: • Pollard C, Bailey KA, Petitte T, Baus A, Swim M, Hendryx M.
