Biographical Sketch:
University of Kentucky Department of Family and Community Medicine:
1)Champion: Academic Chronic Care Collaborative, AAMC (American Association of Medical College. 2005-present
•Coordinating the AAMC collaborative (UK is one of 22 academic centers) whose goal is to improve outcomes in people with chronic conditions such as diabetes.
•Project deliverables: a) Integrating the chronic care model in Department of Internal Medicine and Family Medicine. b) Empower all clinic members (health providers, staff and patients) to improve outcomes. c) Work with faculty on the Pay for Performance issues. d) Collect data on outcomes measures to improve health care delivery. e) Educating and working with representatives of Dean’s office, KMSF, faculty, all clinic employees, residents, nurse practitioners, physicians’ assistants about the chronic care model. f) Recognition of the project by AAMC and invitation to be one of the trainers.
2) Complementary and Alternative Medicine (CAM) grant 2004-2007
•Developed educational training programs for “conventional” and CAM providers who serve as preceptors for students.
•Assisted in the integration of CAM curriculum into medical school, residency, and faculty education.
3) Full time Faculty Department of Preventive Medicine 1997-2004 Responsibilities included: patient care, teaching of faculty, medical students, residents, Pharm Ds, nurse practitioners, physicians assistants. Resource expert for the university’s pilot Pharm D directed diabetes management program
