Biographical Sketch:
April 2008-Present -- Evaluation Coordinator/Graduate Research Assistant -- Diabetes Prevention and Control Program, Bureau of Community Chronic Disease Prevention, NYS Department of Health, Albany, NY Developed, administered, and analyzed a web-based survey to assess the extent to which members of the NYS Diabetes Task Force (the advisory body to the DPCP) reflect diversity in gender and ethnicity, and mirror the epidemiological profile of the burden of diabetes in NYS. Drafted an evaluation plan for the Creating Healthy Places prevention initiative that will establish programs to prevent obesity, type 2 diabetes and other chronic diseases in NYS. Performed diabetes-related research, writing and analysis for reports, publications, abstracts, grants, funding announcements and other DPCP needs. September 2007-March 2008 -- Graduate Intern -- DPCP, Bureau of Community Chronic Disease Prevention, NYS Department of Health, Albany, NY Assisted in the development of a diabetes media campaign and awareness-raising materials for minority populations in Montgomery and Albany counties in NYS Coordinated a NYS Certified Diabetes Educator (CDE) education initiative to train school nurses and personnel on caring for children with diabetes at school – responsible for managing a database of CDEs in NYS and making linkages between schools requesting training and local area CDEs. Conducted project-specific analyses to demonstrate effectiveness of DPCP initiatives, including but not limited to: - An analysis of patient outcome data from a diabetes registry systems improvement project in a private practice in Westchester County, NYS - An analysis of results to a web-based survey used to evaluate the quality and impact of a diabetes prevention and management resource toolkit among health care providers in NYS. April 2003-August2007 -- Program Assistant -- Northeast Health Diabetes Center, Albany Memorial Hospital (AMH), Albany, NY Conducted continuous quality improvement evaluation of the AMH Diabetes Center’s Diabetes Self-Management Education Program – responsible for collecting, analyzing and reporting data on patient outcomes to the American Diabetes Association as a part of the program recertification process. Assisted with all clerical aspects of the Albany Memorial Hospital outpatient diabetes center, including billing, scheduling, marketing/promotion and grant writing.
