Biographical Sketch:
2004 to Present Kentucky Diabetes Prevention and Control Program, Kentucky Department for Public Health, State Staff / Diabetes Quality Improvement Coordinator. Under administrative direction of the Director of the Kentucky Diabetes Prevention and Control Program, responsibilities include directing one or more statewide public health programs related to diabetes prevention and control. This responsibility includes: accessing statewide needs, mobilizing appropriate Local Health Department (LHD) staff and other internal and external partners, facilitation of the planning process, developing budgets for the respective efforts/programs, implementing appropriate statewide interventions, evaluating the efforts/programs, and maintaining effective communication with Center for Disease Control’s (CDC) Division of Diabetes Translation, state diabetes prevention and control staff, local health departments, a wide array of public and private partners (e.g., health plans, voluntary associations, health care providers, professional associations, etc.) as well as the general public. Examples of specific responsibilities have included: organizing and conducting an ongoing statewide diabetes assessment and planning project, development and implementation of a statewide diabetes newsletter (the Kentucky Diabetes Connection) with 10 other diabetes entities, development of a web based statewide diabetes resources directory, and liaison with federally funded Diabetes Collaboratives.
1990 to 2004 Green River District Health Department, Chronic Disease Supervisor -- Activities Listed Below Along With Management of the diabetes, cardiovascular, and cancer control programs within the seven county Green River District including: hiring and training staff, writing program plans, developing a budget, securing physician contracts, evaluating and expanding program protocols.
1984 to 2004 Kentucky Diabetes Prevention and Control Program, Green River District Health Department Diabetes Team -- Organize and facilitate Diabetes Today Community Coalitions-- which includes: community assessment, data gathering, data analysis, recruitment of key community leaders, and an initial 2-day course for the coalition to identify and prioritize diabetes related problems, and develop an intervention strategy to address them; Plan, organize, and coordinate continuing education for health care professionals (nurses, nutritionists, pharmacists, physicians, etc.); Teach patient group classes in the Green River District, a seven county area; Serve as liaison between the seven local health departments' staff and the KY Department for Public Health staff; Work cooperatively with various community agencies and organizations to ensure that comprehensive diabetes services/education are provided in the District.
Presented numerous times at national meetings including the American Association of Diabetes Educators and the CDC Diabetes Translation Conference as well as numerous state programs.
