Coordinate and provide oversight for all Diabetes Prevention and Control Program (DPCP) activities and events
Achieve DPCP goals and objectives as established through CDC cooperative agreement , the New York State Plan for the Prevention and Control of Diabetes and as required by other funding sources
Develop required application and evaluation reports for multiple funding sources, including the CDC, Maternal and Child Health Block Grant and Preventive Health and Health Services Block Grant
Collaborate with DPCP surveillance unit to implement surveillance systems and produce surveillance and burden documents to share with state and federal partners
Coordinate funding sources and develop DPCP budget and expenditure plans
Collaborate with NYS funded contractors including Community Coalitions for Diabetes Prevention, Diabetes Centers of Excellence and Type 2 Diabetes Prevention in Children contractors to implement the national objectives for diabetes prevention and control as established by the CDC and NYS
Communicate with contractors on a regular basis to facilitate sharing of success stories, problem solve, and respond to need for resource development and training including planning and coordination of annual contractors’ meeting
Develop and implement systems to support contractors in their submission of acceptable workplans and evaluation reports
Provide guidance and support to other staff members within the Diabetes Control and Prevention Program, with direct supervision of five professional level staff and one support staff member
Develop Requests for Applications and other funding opportunities to promote diabetes control efforts statewide
Develop policy recommendations to support DPCP goals
Gather input from statewide diabetes and chronic kidney disease stakeholders to guide program direction via the creation and maintenance of advisory bodies, including the NYS Diabetes Task Force and the newly established Chronic Kidney Disease Task Force
Serve as liaison between DPCP and various national diabetes and chronic disease prevention initiatives and diabetes advocacy organizations (American Diabetes Association, National Kidney Foundation, American Association of Diabetes Educators, National Association of Chronic Disease Directors’ Diabetes Council, Juvenile Diabetes Research Foundation, etc.)
Collaborate with other Bureaus and Divisions within New York State Department of Health to further mutual programmatic goals
