Biographical Sketch:
19 years with Migrant Health Service, (Moorhead, MN) coordinating medical care for migrant farm workers and their families. Had a real desire to assist the families with their diabetes issues, so was instrumental in obtaining a startup grant for their diabetes program. Also worked 14 of those years, part-time in a private college health service.
Responsible to coordinate a statewide diabetes quality improvement project in conjunction with the state health department.
Develop and maintain a claims data registry of all Blue Cross Blue Shield of North Dakota diabetes Members within an Access database.
Determine criteria for patient selection. *Diabetes identification (i.e. diagnosis codes) *Assign primary care physician. *Demographics
Establish peer norms from which to make comparisons.
Develop/maintain physician reports assessing the frequency of annual office visits, A1C testing, lipid testing, eye exams, and microalbumin testing or know nephropathy
Mail quarterly diabetes reports to providers in the state of ND.
Responsible to maintain records of contractual agreements with North Dakota Department of Health Diabetes Control Program
