Biographical Sketch:
Donna Zazworsky is the Director of Carondelet Health Network’s Diabetes Care and Community Outreach Centers, a four-hospital network in Tucson and Nogales, AZ. In this position, she is responsible for integrating and bridging diabetes prevention and self-management education activities in acute care, outpatient and community practices.
Ms. Zazworsky has authored numerous articles and chapters on case management, disease management and advanced practice nursing. Most recently, she published the Handbook of Diabetes Management, a resource textbook for community health providers published by Springer.
Ms. Zazworsky is an adjunct Clinical Associate Professor at the University of Arizona, College of Nursing, Past-President of the Beta Mu Chapter of the International Honor Society of Nursing and Chair of the Communication and Publicity Committee for the American Academy of Nursing.
