Biographical Sketch:
2006-Present -- Program Director, Diabetes Prevention and Control Program, Wisconsin Department of Health Services, Madison, WI Directs the DPCP, leads statewide diabetes team, provides day-to-day leadership to the DPCP and manages various contracts with statewide partners, provides clinical consultation and technical assistance to the DPCP, coordinates and staffs the Diabetes Advisory Group and the HMO-Wisconsin Collaborative Diabetes Quality Improvement Project, coordinates new diabetes quality improvement initiatives, maintains and strives to expand statewide partnerships, continues to lead efforts for the Essential Diabetes Mellitus Care Guidelines revision and update, etc. 2002-2006 -- Health System Specialist Public Health Nurse, Diabetes Prevention and Control Program, Wisconsin Department of Health Services, Madison, WI Served as clinical lead for the DPCP; led efforts for the Essential Diabetes Mellitus Care Guidelines revision and update; assisted with other DPCP projects, such as strategic planning process, preparation of required CDC applications and progress reports; collaborated with healthcare professionals, other professional groups and community members to increase awareness of diabetes prevention and control; served as a resource person for quality improvement for the Federally Qualified Health Centers in Wisconsin; coordinated and implemented site visits regarding provider profiles to RN staff for on going reinforcement of diabetes quality care and support services; collaborated with multiple professional organizations regarding diabetes and other chronic diseases to educate and coordinate care for populations at risk 1992-2003 -- Diabetes and Health Educator, Jackson Clinic-Physicians Plus-UW Health, Madison, WI Worked with diverse groups of people living with a chronic disease; provided individualized care to assist with behavior change and adapting to living with a chronic disease; conducted initial and ongoing education for people with diabetes (one to one contact, group classes and community support groups); worked as a team with primary care practitioners to co-manage diabetes regimes to help people obtain goal blood sugar levels and enhance self management skills; educated and promoted standards of care for people with diabetes through individual visits, groups and public speaking; developed and presented various educational programs and workshops for people with diabetes, family members, community members and health care professionals; promoted and advocated for patients to receive quality diabetes care when noted to be neglected Author or coauthor of numerous abstracts relating to diabetes prevention and control.
