Biographical Sketch:
Jay has been the Principal Investigator of the Diabetes Primary Prevention Initiatve with the CDC from 2005-2009. In this role he has been involved with Surveillance, Systems Dynamics Thinking, and Intervention Focus Areas. In Minnesota he has helped formulate our strategies for diabetes primary prevention. He has worked closely with the Minnesota Diabetes Steering Committee and the Surveillance Data Review Work Group through out his tenure with the Minnesota Diabetes Program as and Epidemiologist and Senior Epidemiologist. In addition, he is actively involved with the health care reform activities in Minnesota and a key link for the Minnesota Diabetes Program in working with key partners in the health provider and health systems arena. He has been the lead evaluator on the work with the DPP in Minnesota, the I CAN Prevent Diabetes program, and has been instrumental in completing the IRB, clinic reviews and supervising the data evaluation on this project.
