Established strong working relationships with Native American partners, Department of Health and community programs and with Project ECHO at University of New Mexico to identify and bridge gaps in healthcare provider knowledge about evidence-based methods to elicit behavior change. 2009 – Present.
Established collaborative working relationships between the Diabetes Prevention and Control Program, the New Mexico Primary Care Association and 18 Community Health Centers in New Mexico to assess the need for and to plan quality improvement activities to improve outcomes for people with diabetes. 2009 – Present.
Coordinated a day-long pre-diabetes training targeted to diabetes educators in the State of New Mexico, especially focusing on the areas of pathophysiology and screening, cardiovascular disease implications, treatment guidelines, medical nutrition therapy guidelines and use of motivational interviewing in prevention of diabetes. 2009 – Present.
Provided patient education, facilitated support groups and respite activities for children and families affected by chronic and life-threatening illnesses. 2001 – 2009.
Moderated discussions about ethics in international healthcare with REACH program medical students and residents. 2007.
Provided leadership in community health center quality improvement around diabetes and asthma through work with the Institute for Healthcare Improvement. Implemented the chronic disease model at an urban community health center and presented project findings nationally at IHI conferences and to the Bureau for Primary Health Care in Washington DC. 1998 – 2001.
