Data Quality Techniques 101

BACKGROUND:
ImmuLink, the Twin Cities Metro Regional Immunization Registry was launched in 1995. It consolidates over 800,000 records and 5.5 million immunizations. Providers electronically submit immunization batch data or directly enter data into the registry. ImmuLink has developed standardized procedures to address data errors. Through chart reviews, internal operations, and routine queries, data is reviewed to ensure registry integrity.

OBJECTIVE:
Share tools for implementing a data quality system.
Provide strategies for identifying and addressing human data error, thereby providing an accurate, reliable registry.

METHOD:
ImmuLink utilizes a simple chart review procedure whereby submitted data can be checked for data entry and file discrepancies. Ongoing and periodic queries are conducted to verify submitted data includes required, accurate demographic and immunization information. Communication techniques ensure that data discrepancies are addressed timely.

RESULT:
Initial and ongoing standardized reviews identify vaccines not entered, or entered incorrectly. Errors are found both in how datafiles are created and in data entry into either the registry itself, or into the participating site’s billing system or personal database. Registry identifies possible lost revenue and vaccine administration practice issues. Sites appreciate receiving the information and the opportunity to improve clinical practices.

CONCLUSION:
Human error is inherent in data entry, regardless of whether data is entered directly into a registry, or entered into a clinic billing system or immunization database and provided electronically. By identifying and addressing data entry errors, registries can help participants improve clinic practices, capture lost revenue, accurately and efficiently immunize patients and share information. Standardized procedures are necessary to improve data quality efficiently and effectively.
