Documenting All Doses: Combining Data from Paper and Electronic Records to Support Assessment Activities

BACKGROUND:
The Houston-Harris County Immunization Registry (HHCIR) is a joint public-private endeavor with the purpose of providing accurate, up-to-date immunization information to both public and private health care providers in Houston

OBJECTIVE:
To assess the performance of six Houston Department of Health and Human Services (HDHHS) health centers according to the following measures: children receiving 4 DTaP : 3 Polio : 1 MCV by two years of age and children receiving 4 DTaP : 3 Polio : 1 MCV : 3 Hib : 3 HepB by two years of age.

METHOD:
HDHHS assessment personnel conducted full CASAs at six health centers. The missing list generated from CASA paper record assessment was checked against HHCIR data. The 4:3:1 and 4:3:1:3:3 coverage levels from the paper medical record data (baseline) were compared to the 4:3:1 and 4:3:1:3:3 coverage levels by adding immunizations data found in HHCIR that were listed as missed according to the CASA data.

RESULT:
Assessing only paper records yielded coverage rates for 4:3:1 of 78% and 4:3:1:3:3 of 74% for all six clinics. Researching and adding missing immunizations yielded increased coverage rates for 4:3:1 of 81% and for 4:3:1:3:3 of 79% for all six clinics.

CONCLUSION:
Assessments of both the electronic and paper records at all six clinics yielded a 3% increase in coverage rates for 4:3:1 and a 5% increase in coverage rates for 4:3:1:3:3.

LEARNING OBJECTIVES:
Utilization of electronic records from a registry system coupled with paper records yields a higher percentage of children immunized according to each of the two performance measures than each would separately.
