BACKGROUND:
The Regional Early Childhood Immunization Network (RECIN) is an Internet-based, real-time immunization registry deployed in public and private provider offices in 23 Wisconsin counties. RECIN created a Tiered Intervention System (TIS), which records and tracks individual patients’ intervention histories. It moves patients through an automated intervention process, which prevents duplication of services, and saves time and money for providers. Tiered intervention has been used successfully to raise and maintain immunization rates over 90% UTD in several 0-2 year old populations for the past three years. It is now time to duplicate this success in the 65 years and over population.
OBJECTIVE:
Raise Pneumococcal coverage rates of patient’s age 65 and older to 90% by using RECIN’s TIS.
METHOD:
A team of private providers, quality improvement and registry support staff formed in early 2003 to develop collaborative campaigns to raise Pneumococcal immunization rates through the use of RECIN.
RESULT:
Through chart reviews, QI staff discovered that Pneumococcal coverage in the aforementioned adult population was underreported by nearly 30%. Before patient outreach efforts could be undertaken, policies and goals were established to facilitate the recording of historical immunization information into RECIN. Development staff expanded the TIS to allow for easier identification of this adult population. After a 6-month period of entering historical information, outreach to the adult population began using TIS.
CONCLUSION:
Providers have been able to raise immunization rates when all health care providers work collaboratively towards a common goal. RECIN’s TIS enhanced the collaborative efforts and allowed ease of tracking and outreach efforts.
