Immunization Data Workflow: Paper Scan Forms into a Registry Web Application

BACKGROUND:
Michigan providers are required to report childhood immunization administrations to the Michigan Childhood Immunization Registry (MCIR). Provider sites without internet access use standardized paper scan forms to report immunization data.

OBJECTIVE:
To give providers without internet access a method for entering immunization data into the registry.

METHOD:
Providers fax paper scan forms to the MCIR Scan Center. Scan form data undergoes manual error correction before being converted into an ASCII fixed format text file. The text file is then securely uploaded to the registry utilizing the MCIR Web Application.

RESULT:
Approximately 16,000 shots are entered into MCIR every month on behalf of 100 provider sites: this represents 5 percent of the total locations reporting to the registry. Five of the six MCIR Regions utilize scan forms.

CONCLUSION:
Scan forms are an alternate method for providers to enter data into an immunization registry.
