D9: READII ROCHESTER: The Translation of Adult Guidelines in the Community

READII ROCHESTER is designed to reduce the disparity in immunization rates between White and African American adults using community outreach and office-based interventions including tracking, reminder/recall, and outreach to patients. In this workshop, we will describe the phases of the project, and explore how results can be applied to other settings to improve the application of clinical and public health guidelines to community health improvement.
Learning Objectives: After attending this workshop, participants should be able to identify successful and synergistic strategies to translate and implement adult immunization guidelines in community health centers.
