Increasing Adult Pneumococcal Immunization Rates Through Innovative Software Applications and Collaberative Efforts

Learning Objectives for this Presentation:

By the end of the presentation participants will be able to:
1. Identify key factors in raising pneumococcal immunization rates among adults age 65 years and older.
2. Identify under-immunized patients.
3. Intervene using tiered intervention strategies.
4. Coordinate a system wide collaborative effort to raise immunization rates.


Background:

The Regional Early Childhood Immunization Network (RECIN) is an Internet-based, real-time immunization registry deployed in public and private provider offices in 23 Wisconsin counties. RECIN created a Tiered Intervention System (TIS), which records and tracks individual patients' intervention histories. It moves patients through an automated process, which prevents duplication of services, and saves time and money for providers. Tiered intervention has been used successfully to raise and maintain immunization rates over 90% YTD in several 0-2 populations for the past four years. RECIN is now being utilized to duplicate this success in adult populations.


Setting:

Marshfield Clinic, one of the largest private multi-specialty group practices in the United States, developed and utilizes RECIN along with Preserv an in house developed preventative care application.


Population:

Patients of Marshfield clinic who are age 65 and over or have a diagnosed disease state such as Diabetes Type I, who are due for the pneumococcal immunization.



Project Description:

Integrating appropriate components of Marshfield Clinic's robust electronic medical record in conjunction with an immunization registry (RECIN) and a preventative care module (Preserv) to provide accurate assessment, identification and intervention for the patients who are due for a pneumococcal immunization.


Results/Lessons Learned:

Through a team approach, providers have been able to raise pneumococcal immunization rates to over 90% in adult populations when all health care providers work collaboratively towards a common goal. The use of RECIN and Preserv enhance collaborative efforts and allow for ease of tracking and outreach efforts.
