C3: C-3 Risk Assessment Screening and Tools

It is of interest to identify effective and efficient ways to integrate preconception and interconception care into clinical settings and existing programs. This session will provide insights on the acceptability and utility of integrating reproductive planning questions into primary health care encounters for female and male outpatients. Participants will also learn about a Problem Knowledge Couplers tool that can be customized for specific populations and used for preconception care clinical screening, risk assessment, intervention and patient education. Application of this tool to augment an existing interconception teen case management program for pregnant and parenting teens (“Teen Care”) will be discussed. This will illustrate the potential for integrating preconception care into existing public health and social service case management and home visiting programs.
Learning Objectives: Participants will be engaged to explore the integration of preconception care into primary health care encounters and corresponding insights into provider and client views on reproductive life planning delivered in the primary care setting. In addition, participants will learn the steps to take to incorporate preconception care into an existing interconception care case management program for pregnant and parenting teens or “Teen Care”. Preliminary teen behavioral outcomes will be shared. Finally, participants will learn about a customizable Problem Knowledge Couplers tool for preconception clinical screening, risk assessment, intervention and patient education. Application of this tool to the “Teen Care” program will illustrate implementation of this tool.
