The CDC Communities of Practice Program (CoPP) aims to foster the development of Communities of Practice as an effective and efficient method for collaborative problem solving and innovation. As new domain areas for development of PHIN communities are identified, a clear process and proven resources are needed by new community champions to assess potential community readiness, plan and launch the CoP, evaluate its effectiveness, and sustain and evolve the community over time. In support of this need, the CoPP has developed a Community of Practice Toolkit. The Toolkit contains resources, templates and tools to provide support for a group at all stages of its lifecycle, along with reference information on the communities of practice theoretical groundwork upon which the Toolkit is built. This presentation will outline the benefits of using the CoP Toolkit to launch a new CoP or to support an existing one. It will describe the resources and tools packaged in the CoP Toolkit and explain how best to use them.
