The goal of the Kentucky community surveillance project is to improve reportable disease communication between local hospitals and public health.  Using locally developed interface engine technology, identity management tools, free GIS tools (Google Earth) and CDC developed software (EARS) Kentucky communities have been able to establish near-real time, hospital-based reporting of infectious diseases.  This presentation will focus on the collaboration, legal, financial and technical challenges related to this project and demonstration of the technology.
