22106 Mobile Diabetes Wellness Intervention

Objective:          The objective of this health initiative is to address health disparities among the high-risk populations by reducing non-compliance rates, particularly in the areas of A1C and lipid testing, dilated eye exams, and foot exams. The goals of the program are to increase the targeted population’s knowledge about the importance of disease management and control and to increase their utilization of standard diabetes exams and/or tests (e.g. total cholesterol, HDL cholesterol, glucose, A1c, and blood pressure).           

Methods:                    The intervention applies evidence based methods for improving A1C, Blood Pressure and lipid control among expo participants. This initiative targets underserved adult populations by zip code who live in Delaware’s rent-assisted communities, are diagnosed with diabetes, and who are non-compliant for at least one year in at least one of three diabetic screenings.

Results: An intake form was developed so each participant in the program is identified and evaluated.  The form collects individual information including insurance, family physician, screening services obtained, and screening outcomes.  Following the mobile wellness expo screening session, screening results are delivered to the participant’s family practitioner, the participant, and a copy is maintained for program evaluation. 

Conclusion: A report system tracks the facilities participating in program, individual residents participating in program, and outcomes of their participation.
