Objective:To describe the evaluation framework and local surveillance strategy for the Indiana Women's Diabetes Initiative

Methods:Clients who enroll in the IWDI are assessed on the status of their diabetes self-management behaviors, tobacco use, level of physical activity, and body mass index at baseline. Diabetes navigators then connect each client with free or low cost resources within their community. After one year, the status of each client’s diabetes self-management behavior and other indicators are formally assessed during a scheduled follow-up appointment.

Results:Fewer of the IWDI clients enrolled in the targeted areas met the annual A1C, annual eye examination, annual foot examination, annual dental objectives Fewer of the IWDI clients enrolled in the targeted areas met the diabetes education and daily blood glucose check Fewer of the IWDI clients enrolled in the micropolitan area and more of the clients in the small metropolitan area were overweight and obese Fewer of the IWDI clients enrolled in the micropolitan area and more of the clients in the small metropolitan area engaged in leisure time physical activity

Conclusion:The IWDI program is reaching women living with diabetes in each county; however, only about a third of the clients have returned for follow-up at 12 months as planned. Among those returning for follow-up, there was improvement in each diabetes self management objective area.
