22198 Delaware DPCP's Utilization of Electronic Medical Records in Health Centers

Objective: To customize health center’s EMR for utilization by a DPCP

Methods: The Delaware DPCP partnered with 3 Federally Qualified Health Centers (FQHCs) and the Delaware Health Information Network (network and software specialists) to create a unique diabetes education matrix in the Electronic Medical Record (EMR) utilized by the FQHCs. The group customized the  EMR to provide a standardized format  for patients with diabetes.  Staff collaborated to design the diabetes education visit matrix and also to train diabetes educators. The group began customization of the EMR in early 2009 in preparation for implementation of the EMR in July 2009.   Critical, early decisions were made to develop the diabetes education sections of the EMR. 

Results: The DPCP can access data related to diagnoses, reason for visit, social and health maintenance history, patient education topics and follow-up needs.  Contractual diabetes educators’ activities can be audited via detailed work logs in the EMR, thereby standardizing documentation.   

Conclusion:    The results will position the group to fulfill projected “Meaningful Use” documentation requirements and better monitor patient services. The DPCP was fortunate to be included in the EMR customization, as all input is time sensitive. Critical, early decisions are necessary to develop the diabetes education sections of the EMR. Later involvement would not have produced these results.  Instead, common goals of the DPCP and FQHCs  related to optimal patient care, practice efficiency, CQI and new reporting capability have been met. The DPCP looks forward to continued collaboration, improved patient care and comprehensive reports as a result of this strategic partnership.
