22357 Pre-Diabetes Basket of Care and Certifying DPP Community-Based Programs: A Perfect Storm of Prevention Opportunity

Objective:An overview of the 2008 actions of the Minnesota Legislature which established the creation of Baskets (bundles) of Care, the subsequent advocacy for, and development of a Pre-Diabetes Basket. This represents one of the first reimbursement models for pre-diabetes, and has significant opportunity when paired with the DPP-styled community-based interventions being implemented in the state.

Methods:The Pre-Diabetes Basket was largely developed through utilization of the Standards of Care of the American Diabetes Association and the 16-week curriculum of the Diabetes Prevention Program.  Work is continuing to promote purchaser, payer, provider and patient utilization of the Basket, for implementation beginning in January, 2010.  How-to manuals for implementation are being created, with an emphasis on partnerships with DPP-styled, community-based programs.  Additionally, progress is being made on the certification of trainers of the DPP-styled, community based programs, which afford an opportunity for the creation of interventions  which will hopefully meet insurer standards for reimbursement.

Results:This session will provide an overview of the process of advocating for, and creation of the Pre-Diabetes Basket of Care, adoption and implementation progress to-date, DPP-trainer Certification legislation and progress, and known levels of patient participation and progress.

Conclusion:The pairing of a Pre-Diabetes Basket of Care with DPP-styled intervention represents one of the first and best opportunities to fund an effective program of diabetes prevention.
