The Role of Michigan’s Childhood Immunization Registry (MCIR) in AFIX Provider Quality Assurance Reviews

BACKGROUND:
Michigan conducts annual AFIX visits of provider offices. Since 2002, Michigan’s AFIX intervention has included a Quality Data Comparison (QDC) enhancement.

OBJECTIVE:
To describe Michigan’s AFIX-QDC process so that other programs can evaluate whether participation in similar efforts will benefit their program.

METHOD:
QDC compares the immunization dates recorded in a patient’s chart with those in the MCIR. Dose dates not documented in the registry are added into the registry. Registry immunization records are printed to show providers the vaccine dates not found in the patient’s charts. CASA is updated with the additional dose dates found in the registry. Data is compiled into a comprehensive report and shared with the provider office.

RESULT:
To date, the QDC process has added over 32,000 dose dates to the MCIR and identified an additional 32,000 dose dates in the MCIR that were not documented in the patient’s chart.

CONCLUSION:
QDC gives practices a more accurate depiction of their patients’ immunization histories (practice coverage levels increased, with a range of 15%– 45%). As a result, practices tend to me more accepting of the AFIX results. More importantly, the results demonstrate to the provider the benefit of accessing MCIR, in order to get a more complete immunization status for their patients, save the practice time, and reduce over-immunization.
